Declaration of consent to Stellar Information Technology Private Limited (Stellar)

SUBJECT: REQUEST FOR ACCEPTING TAMPERED STORAGE MEDIA

I, hereby request Stellar to accept my storage media for Data Recovery Attempt purpose. I, declare that my storage media has already been tampered by another vendor earlier in a non-confirmatory environment severely affecting its data recovery chances.

I am now aware that confirmatory environment like CLASS 100 CLEAN ROOM Lab is essential to open this Storage media in order to get the Data Recovery Results.

Although Data recovery chances are remote from the above-mentioned Storage Media, Since the Data in the affected media is very critical & important for me, I hereby request Stellar Data Recovery to undertake an attempt for DATA Recovery.

I am ready to pay the required ATTEMPT FEE which I accept is non-refundable and payable in advance. I further agree to the condition that the results MAY or MAY NOT be in favor of a successful Data Recovery as required by me. I hereby affirm that Stellar Data Recovery will have no liability for the resulting outcome in this case what-so-ever.

In case of Successful Data Recovery, I accept the recovery cost (which is separate and in addition to Attempt Fee).

I agree that for my submitted tampered media the condition of “no recovery no charge” will not apply ; further , I agree to all the Terms and Conditions as specified in Stellar’s Media Assessment Form (MAF).

I request Stellar to accept my above request and kindly proceed ahead with attempt process accordingly.
